Effective use of Bronchoscope

Bronchoscopy is an endoscopic process of seeing the insides of the airways for diagnostic and therapeutic reasons through a bronchoscope, which is inserted via nose or mouth. In 1897, Gustav Killian performed the first bronchoscopy using a rigid bronchoscope to remove a pork bone. Shigeto Ikeda, invented the flexible bronchoscope in 1966.

Bronchoscopy is used for several purposes like seeing the abnormality in the airways, obtaining tissue from the lungs through biopsy or endobronchial brushing or bronchoalveolar lavage, to remove foreign objects or secretions or blood from the airways, stent placing to relieve the pressure on tracheobronchial lumen from malignant or other diseases. Also, Bronchoscopy is used in percutaneous tracheostomy and tracheal intubation of patients.

Bronchoscopy is usually done in OT or ICU or in places capable of managing airway emergencies. The patient is given anti-anxiety, anti-secretory medications along with analgesics and local anaesthetics to prepare the airways for the process. Flexible bronchoscope is thinner and longer than a rigid bronchoscope and contains a fiberoptic system in the tip which sends image to the video camera or eyepiece at the other end. Rigid bronchoscopy is done under general anaesthesia. Flexible bronchoscopy causes less discomfort to the patient. ECG, BP and pulse oximetry monitoring is done before, during and after the procedure. The bronchoscope is inserted through the nose or mouth of the patient who is kept in sitting or supine position. The device is taken down to the upper airways, vocal cords, trachea and ultimately into the bronchial system and the diagnostic or therapeutic procedure is done.

As recently as March 29, 2017, it was reported by PGIMER’s pulmonary medicine clinic doctors that there is a sudden spurt in the number of bronchoscopy cases since last 3 months. The department which usually did around 10 cases per day is suddenly doing 30 cases per day. They have reported to Union Ministry and soon an investigation on the cause will be done.
